DALLAS, March 4, 2021—Signify Health, Inc. (NYSE: SGFY), a leading value-based healthcare platform that leverages advanced analytics, technology and nationwide healthcare provider networks, and Aspen Physician Network, a high-quality integrated network of specialty physician groups in Texas, announced today that Episodes of Care programs will be available to large and mid-sized employers across the Dallas-Fort Worth region. The programs align financial incentives with the health outcomes of members undergoing medical procedures or managing chronic diseases.

Episodes of Care programs are all-inclusive, value-based payment models where a single, bundled payment includes all services associated with the treatment for an illness, condition or medical event over a fixed period of time rather than a separate fee-for-service model. Through this model, high-performance networks such as Aspen Physician Network are contracted to coordinate care and share accountability for health outcomes.

As part of its role as program administrator, Signify will partner with Aspen Physician Network to activate a suite of advanced technology, analytics, and clinical support services to holistically manage and support the treatment of dozens of chronic medical conditions and procedures:

New effort will improve patient care quality, increase efficiency and lower costs with a central patient record

HASA, the non-profit, community HIE for North, South and West Texas, and Aspen Physician Network, an independent specialty physician group in Dallas-Fort Worth, Texas, have announced that HASA will be the technology foundation for a new clinically integrated network (CIN), which will reduce costs, increase efficiencies, and improve the quality of care for Aspen Physician Network’s patients.

HASA will serve as the platform for the CIN by allowing multiple independent practices in the specialty Aspen Physician Network to have access to and share one patient record. This will help with transitions of care, enhance care coordination and drive future efforts in telehealth and remote patient monitoring.

“Having a central patient record is critical for launching a CIN,” said Danny Conaway, Executive Director of Aspen Physician Network. “Working with HASA will provide a single interface for coordinating and improving all levels of care, and will be key to our move into the value-based care arena.”

Genesis Physicians Group Partners With Specialty Group for Comprehensive Care

Genesis Physicians Group is partnering with a network of specialty physicians to better manage care as patients move between primary physicians and specialists. The relationship with Dallas-based Aspen Specialty Physicians Network is meant to reduce healthcare costs and improve coordination of patient care.

The co-management process will utilize “smart referral” technology and will streamline the treatment of chronic disease conditions. Genesis currently has over 500 primary care and 1,000 specialty physicians members and will add Aspen’s 400-plus specialists in an effort to be successful in contracts with more risk while expanding the ability of primary care physicians to stay independent and care all their patient’s needs.
